Request for Preparing the List of Participants

For the Joint Usage/Research Center Program of OCAMI at Osaka Metropolitan University, reporting to MEXT (Ministry of Education, Culture, Sports, Science and Technology) is mandatory. Therefore, for workshops, conferences, and other events held as part of the OCAMI Joint Usage/Research activities, we kindly ask you to prepare a list of participants including the information below and submit it to the OCAMI office after the event. (The data items were revised on April 24, 2025.)

      • Name
      • Affiliation
      • Position
      • Category (Student, Non-Student (Age 35 or Younger), Non-Student (Age 36–39), Others)
      • Gender
      • Participation Method (Venue Participation, Online Participation, or Both)
      • Participation Date(s)

The items “Category” and “Gender” are defined based on the items required in the report submitted to MEXT.

Registration Form Template

It is convenient to create a registration form using the Microsoft Forms template below. Please prepare a Microsoft account for the organizer, duplicate the template below, and replace the contents with information for your conference or other event before use. (You may add additional questions as needed, such as whether participants wish to give a talk.) After the event, export the responses from the form as an Excel file and submit it to the OCAMI office.

Handling of Personal Information

The personal information contained in the list of participants you provide will be appropriately managed by OCAMI as described below.

1. Restrictions on the Use and Provision of Personal Information

The personal information provided will be used only within the scope of the purposes clearly specified in advance and will not be used or provided for other purposes, except when the consent of the individual has been obtained or when required by laws and regulations.

2. Purpose of Use

The information provided will be used to compile statistical data to be submitted to public institutions such as MEXT (Ministry of Education, Culture, Sports, Science and Technology). Since the data will be aggregated numerically for each item, no personally identifiable information will be submitted. Names and email addresses may be used to contact participants to confirm the accuracy of the information listed in the list of participants.
